What is the process for leagally changing a child's name?

I would just like to add an additional name to their middle names. I want to add in my maiden name but not hyphenate.

Welcome to the Office of the Registrar General's Change of Name Wizard


The process for legally changing a person's name can be lengthy. There are several different forms that may be applicable depending on the circumstances. For instance, you may wish to take a spouse or conjugal partner's name, revert to a previous name, change a child's name or formally change an adult's name.





The Change of Name Wizard will help select the right process for you. The Wizard will ask you a series of questions and then will either identify the appropriate form for you to complete, or direct you to other ways to change a name. Once directed to the proper form, please print and complete the form and mail to the Ontario Office of the Registrar General with the appropriate fee for processing.





A person's name is his/her identity and changes to a name must be recorded accurately with the Ontario Office of the Registrar General to safeguard against identity fraud.

Minimum PC browser requirements:


- Microsoft Internet Explorer version 5.5 or higher - press here for a newer version.

Minimum Mac requirements:

Reply:different states have different requirements. some even charge fees for it. go to this website, it should give you all the info, forms, and places that you need to go. best of luck!

Reply:same as for an adult, silly goose. ask me another question.
Reply:I don't know what state you are in, or exactly what your situation is, but my daughter's last name is hyphenated (obviously my last name and her dad's last name) and it hasn't always been that way. I just changed it by ammending her birth certificate. Hope this helps! Good luck.
